Gala Day - Soccer Boys - Semester 1 2020

Congratulations on being chosen as a Sports Representative for Yeronga State School. You will be representing your school in the Sunnybank Inter-School Sports District and must be conscious of upholding the school ethos at all times. Please ensure you have read and understand the two documents attached to the email; 1. Yeronga State School Sporting Contract & Activity Consent Form and 2. Sports Gala Day Note.

In the event of an accident or illness, I authorise school staff to obtain or administer any medical assistance or treatment my child may reasonably require, including contacting my child’s doctor.

I have provided the school with all relevant details relating to my child’s medical or physical needs on enrolment and where relevant have updated this information.

I accept liability for all costs incurred in obtaining medical assistance or treatment (including any transport costs) & undertake to reimburse the Department of Education the full amount of any costs incurred on my child's behalf.
